Role Summary

The Bridge is dedicated to enhancing lives by providing essential support, hope, and opportunities to the most at-risk individuals in our community. Our organization offers a comprehensive range of evidence-based rehabilitative services, including mental health and substance abuse treatment, housing assistance, vocational training, healthcare, education, and creative arts therapies.

As a Case Manager at The Bridge, you will play a crucial role in assisting formerly homeless individuals diagnosed with severe mental health issues and/or substance use disorders. These clients reside in independent living situations, either in a single-site facility or dispersed throughout the community. Your primary responsibility will be to deliver vital support services that enable these individuals to achieve stability and reach their personal aspirations.

Key Responsibilities:
  • Deliver services primarily in clients' residences and community environments.
  • Conduct assessments and facilitate referrals for high-risk clients, providing case management and interventions.
  • Identify clients' strengths, needs, and goals, and develop corresponding Support Plans within the electronic client records system.
  • Utilize Motivational Interviewing techniques to empower clients in developing skills necessary for maintaining housing stability.
  • Ensure timely documentation of all client interactions in compliance with agency policies and regulatory standards.
  • Co-facilitate group sessions on topics such as wellness, leadership, housing stability, employment, education, and healthy decision-making.
  • Assist clients with housing applications, referrals to medical and mental health services, and coordination of care with external providers.
  • Participate in case conferences with the High Risk Coordinator to strategize interventions.
  • Attend required staff meetings, supervision sessions, and training opportunities.
  • Support program compliance during audits by managing admissions and discharges, maintaining accurate records, and overseeing documentation.
  • Collaborate with various departments, external agencies, and treatment providers.
  • Engage with clients, families, and community members respectfully, acknowledging cultural differences.
  • Conduct apartment inspections across multiple floors and provide client escorts using public transportation.
  • Report serious incidents and sensitive situations to supervisors promptly.
  • Complete incident reports in accordance with agency policy.
  • Account for clients, perform diligent searches, and file missing person reports as necessary.
  • Provide crisis intervention when needed, employing de-escalation techniques.
  • Undertake additional responsibilities as assigned, including potential support for other agency programs.
Qualifications:

Essential Skills and Experience:
  • Bachelor's Degree is mandatory.
  • Experience in mental health, crisis intervention, and trauma-informed care is essential.
  • Strong organizational and time management skills.
  • A collaborative team player who is adaptable, creative, and receptive to feedback.
  • Willingness to be trained in CPR/First Aid/AED/Naloxone.
  • Familiarity with AWARDS software is preferred.
  • Bilingual candidates, particularly in Spanish, are encouraged to apply.
  • Experience working with Veterans or knowledge of Veteran services is advantageous.
