Position Summary
The Bridge is dedicated to improving lives by providing essential support and opportunities to individuals facing significant challenges in our community. Our organization offers a comprehensive range of evidence-based rehabilitative services, including mental health and substance abuse treatment, housing assistance, vocational training, healthcare, and creative arts therapies.
As a Case Manager at The Bridge, you will play a crucial role in assisting formerly homeless individuals diagnosed with serious mental health issues and/or substance use disorders. These clients reside in independent living situations, either in a centralized location or dispersed throughout the community. Your primary responsibility will be to deliver vital support services that enable these individuals to maintain stability and reach their personal aspirations.
Key Responsibilities:
- Deliver services primarily in clients' residences and community environments.
- Provide case management and interventions for high-risk clients, including conducting assessments and facilitating referrals.
- Identify clients' strengths, needs, and goals, and develop corresponding Support Plans within the electronic client records system.
- Utilize Motivational Interviewing techniques to assist clients in developing skills necessary for housing stability. Encourage the establishment of natural support systems and incorporate these into Support Plans and client meetings.
- Ensure timely documentation of all client interactions in accordance with agency policies and regulatory standards.
- Co-facilitate groups as needed in areas such as wellness, leadership, housing stability, employment, education, relationships, and decision-making.
- Provide comprehensive case management services, including assistance with housing applications, referrals to medical and mental health services, and coordination of care with external providers.
- Participate in case conferences with the High Risk Coordinator to develop intervention strategies.
- Attend required staff meetings, supervision sessions, and training programs.
- Assist in preparing for audits by ensuring compliance with program requirements, including managing admissions and discharges, maintaining accurate records, and overseeing documentation.
- Collaborate with other departments, external agencies, and treatment providers.
- Work respectfully with clients, families, community members, and staff, demonstrating cultural sensitivity.
- Conduct apartment inspections across multiple floors and provide client escorts within the community using public transportation.
- Report serious incidents and sensitive situations to supervisors immediately.
- Complete incident reports in accordance with organizational policies.
- Account for clients, conduct diligent searches, and file missing person reports as necessary, adhering to policy and procedure.
- Provide crisis intervention when needed, employing de-escalation techniques.
- Perform additional duties as assigned, which may include supporting other programs within the agency.
Essential Skills and Experience:
- Bachelor's Degree is required.
- Experience in mental health, crisis intervention, and trauma-informed care is essential.
- Strong time management and organizational abilities.
- A collaborative team player who is adaptable, creative, and receptive to feedback.
- Willingness to obtain CPR/First Aid/AED/Naloxone certification.
- Familiarity with AWARDS software is preferred.
- Bilingual proficiency in Spanish is advantageous.
- Experience working with Veterans or knowledge of Veteran-related issues is preferred.
