The Care Management Supervisor is a key member of the Hackensack Meridian Health team, responsible for overseeing the care management process and ensuring that patients receive high-quality, efficient care. Key Responsibilities- Collaborative Care Management: Facilitate the management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
- Staff Supervision: Supervise staff with all care management processes, including length of stay, throughput, patient flow, and denials and appeals follow-up.
- Process Improvement: Apply process improvement methodologies in evaluating team member documentation and ensure that multidisciplinary rounds and team huddles are occurring.
- Performance Management: Develop performance improvement plans for care managers, document performance, and provide performance feedback, evaluate the work of team members, and provide reward and recognition for proper and efficient performance.
- Regulatory Compliance: Participate in departmental preparation for regulatory visits and compliance audits, and ensure that care meets evidence-based practice standards and regulatory/payor requirements.
- Education: Bachelor's degree, Nursing or Master's Degree in Social Work.
- Experience: At least 3 years full-time experience in an acute care setting.
- Skills: Effective decision-making/problem-solving skills, demonstration of creativity in problem-solving, and influential leadership skills, excellent verbal, written, and presentation skills, moderate to expert computer skills.
- Licenses and Certifications: NJ State Professional Registered Nurse License or NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
