Overview

The individual in this role will be tasked with evaluating intake referrals for the Department's preventive services, facilitating connections between families and service providers, and ensuring accurate tracking of referrals through spreadsheets. This position involves collaboration with the DFS Intake and Investigation Program Team.

Key Responsibilities

The following responsibilities are essential and represent core functions typical of all roles within this classification. They are not intended to be an exhaustive list of all duties associated with any single position.

Advocates for the health, safety, and rights of clients and their families, assisting them in accessing necessary services. Conducts interviews with clients and family members or professionals to collect personal, social, and background information; collaborates closely with clients and families to clearly define their challenges; identifies key factors contributing to these challenges; uncovers available resources and strengths; and recognizes barriers that may hinder resolution without state agency intervention. Evaluates eligibility for various social service programs provided by the agency or available within the community. In partnership with clients and families, formulates and executes service/treatment plans that outline goals, methods, and expected outcomes; aids clients and families in securing community, social, and therapeutic services necessary to achieve these objectives. Facilitates individual and group counseling sessions with clients and family members to support effective social development and adjustment to specific challenges. Monitors progress towards established goals and makes recommendations regarding plans such as reclassification, discharge, aftercare, emergency protective placements, guardianship, or options counseling. Prepares comprehensive social histories, client progress reports, evaluations, discharge summaries, and court documents; maintains a variety of records and files related to assigned clients. Enforces court mandates and conditions of community supervision as necessary. Participates in court hearings and other judicial proceedings to provide insights, recommendations, and progress updates regarding clients. May be required to intervene in crisis situations, including potential suicides, assaults against clients, and cases of sexual abuse. Coordination with law enforcement and emergency personnel may be necessary. Physical intervention may be required in certain circumstances. May need to perform emergency duties, which could involve being on call during designated periods.

Qualifications

Professional experience in health or human services may encompass determining eligibility for community resources; connecting clients and families to essential services; addressing family conflicts and facilitating crisis resolutions; and evaluating progress towards goals.
Minimum Qualifications for Family Service Specialist
Candidates must demonstrate competence in the following areas:
1. Candidates must meet at least one of the following criteria: Three years of experience in professional health or human services. OR An Associate's degree coupled with two years of experience in professional health or human services. OR A Bachelor's degree or higher.

Additionally, candidates must fulfill the following requirements:

One year of experience in health or human services with a focus on family safety and welfare, including identifying issues, barriers, and goals; determining eligibility for community resources; assisting clients and families in accessing necessary services; resolving family conflicts; and assessing progress towards goals.

3. Two years of experience in case management, which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individual human service needs.
