Overview
The individual in this role will be tasked with evaluating intake referrals for the Department's preventative services, facilitating connections between families and service providers, and ensuring accurate documentation of referrals. This position involves direct collaboration with the Intake and Investigation Program Team.Key Responsibilities
The key responsibilities outlined below are essential functions that are common across all roles in this classification and are not intended to be an exhaustive list of duties for any single position.
Provides advocacy services to promote the health, safety, and rights of clients and their families, assisting them in accessing necessary services. Conducts interviews with clients and family members, as well as professionals, to collect personal, social, and background information; collaborates closely with clients and families to develop a comprehensive understanding of their challenges; identifies critical factors contributing to these challenges; uncovers available resources and strengths within the family; and recognizes barriers that may hinder resolution without state agency intervention. Evaluates eligibility for various social service programs offered by the agency or available within the community. In partnership with clients and families, formulates and executes service/treatment plans that outline specific goals and objectives, methodologies, and expected outcomes; assists clients and families in securing community, social, and therapeutic services necessary to achieve these goals. Facilitates individual and group counseling sessions with clients and family members to support satisfactory social development and address specific challenges. Monitors progress towards established goals and provides recommendations regarding plans such as reclassification, discharge, aftercare, emergency protective placements, guardianship, or options counseling. Prepares comprehensive social histories, client progress reports, evaluations, discharge summaries, and court-related documents; maintains a variety of records and files pertinent to assigned clients. Enforces court mandates and conditions of community supervision as required. Participates in court hearings and other judicial proceedings to provide insights, recommendations, and progress updates regarding clients. May be required to intervene in crisis situations, including potential suicides, assaults against clients, and cases of sexual abuse. Coordination with law enforcement and emergency personnel may be necessary. May also involve isolating or physically restraining clients exhibiting disruptive behavior. May be called upon to perform emergency duties requiring on-call availability for designated periods.Qualifications
Professional experience in health or human services may encompass, but is not limited to, assessing eligibility for community resources; connecting clients and families to essential services; addressing family conflicts and facilitating crisis resolutions; and evaluating progress towards established goals.Requirements for Family Service Specialist
Candidates must demonstrate education, training, and/or experience reflecting competence in the following areas:
1. Candidates must meet at least one of the following criteria:
Three years of experience in professional health or human services. OR
An Associate's degree coupled with two years of experience in professional health or human services. OR
A Bachelor's degree or higher.
Additionally, candidates must fulfill all job requirements listed below:
2. One year of experience in health or human services focusing on family safety and welfare, including identifying issues, barriers, and goals; assessing eligibility for community resources; assisting clients and families in obtaining necessary services; resolving family conflicts; and evaluating progress towards goals.3. Two years of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individual human service needs.
