Job Summary

Trinity Health is seeking a dedicated Patient Care Provider to join our team. As a key member of our healthcare team, you will provide direct patient care and perform specified procedures under the direct supervision of a registered nurse or licensed practical nurse.

Key Responsibilities
  • Provide high-quality patient care according to established policies and procedures.
  • Communicate effectively with patients, families, and healthcare team members.
  • Assist with patient transfers, ambulation, and other mobility needs.
  • Document patient care delivered and report any concerns to the appropriate personnel.
  • Perform other related duties as assigned or as workload or staffing variations require.
Requirements
  • CNA license preferred.
  • CPR certification on hire or able to attain within 60 days.
  • Experience in patient care preferred.
Work Environment

This role requires the ability to work in a fast-paced environment and respond to changing patient needs. You will be required to wear personal protective equipment and follow infection control protocols.
