About the Role

We are seeking a highly skilled and compassionate Senior Patient Care Manager RN to join our team at Heartland Hospice. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care, and leading a team of dedicated healthcare professionals.

Key Responsibilities
  • Site Management: Oversee the daily operations of our hospice sites, ensuring seamless delivery of patient care services.
  • Clinical Leadership: Provide clinical guidance and support to our nursing staff, ensuring they have the necessary resources and expertise to deliver exceptional patient care.
  • Team Management: Lead and manage a team of healthcare professionals, providing coaching, mentoring, and performance feedback to ensure they meet our high standards.
  • Quality Improvement: Collaborate with our quality improvement team to identify areas for improvement and implement strategies to enhance patient care and outcomes.
  • Communication: Serve as a liaison between our hospice sites and our corporate office, ensuring effective communication and collaboration to achieve our goals.
Requirements
  • Education: Bachelor's degree in nursing (BSN) from an accredited nursing program.
  • Experience: Minimum of three years of experience in hospice or home care, with a proven track record of leadership and clinical expertise.
  • Licensure: Current RN licensure in the state of employment.
  • Certifications: CPR certification and valid driver's license.
