Job Summary

We are seeking a highly skilled and compassionate Integrated Care Paramedic to join our team at Emergent Health Partners. As a key member of our healthcare team, you will play a vital role in providing comprehensive assessments and interventions to patients with acute and chronic medical issues in various settings.

Key Responsibilities
  • Conduct thorough medical assessments and interventions under medical director guidance and care team coordination.
  • Manage established plans of care, evaluate efficacy of interventions, and provide effective coaching and advocacy to progress goals forward.
  • Provide disease-specific education to patients, family, and/or caregivers.
  • Review medical records, treatments, screenings/tests, and coordinate with members of the patient's care team.
  • Triage medical concerns and establish effective plans to ensure patient needs are met.
  • Maintain precise documentation through written reports using Electronic Medical Records.
  • Attend training sessions as required to maintain certification and develop additional community paramedic skills.
Requirements
  • Proficient knowledge of emergency care, common illnesses, symptoms, and medical terms.
  • Ability to make sound decisions independently and collaboratively, even in high-stress situations.
  • Skilled in preparing comprehensive written reports.
  • Effective time management and organizational abilities.
  • Competent in creating educational materials and delivering training as needed.
  • Current Paramedic Certificate for the State of Michigan, or current National Registry.
  • Current certification in CPR and ACLS.
  • Valid State of Michigan motor vehicle operating license.
Work Environment

While performing the duties of this job, the associate is required to stand, walk, use hands and fingers, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
