Position Overview:

The Clinical Transition Educator plays a pivotal role in delivering clinical education and training to facilitate the seamless transition of patients into the Option Care Health service model.

This position collaborates closely with the sales team to enhance the volume of patients transitioning to our care delivery model.

Key Responsibilities:

  • Assess, instruct, and train patients, caregivers, and facility personnel on the integration of Option Care Health services and products in alternative settings, whether in-home or virtually, to ensure effective patient transitions.
  • Conduct thorough patient evaluations to determine suitability for the Option Care Health service model.
  • Collaborate with the sales team to identify educational needs related to clinical transitions and implement strategies to boost patient transition rates.
  • Uphold the confidentiality of patient and proprietary information, adhering to legal standards for safeguarding sensitive data.
  • Document educational interactions accurately using Option Care Health's applications and technology.

Supervisory Role:

This position does not entail supervisory responsibilities.

Essential Qualifications:

  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.

Preferred Skills:

  • Proven ability to establish and maintain relationships across various organizational levels and with external partners.
  • Knowledge of standard practices for all services provided, along with relevant compliance standards.
  • Experience in delivering exceptional customer service to both internal and external stakeholders, ensuring high-quality service delivery and customer satisfaction.
  • Proficient in basic computer skills, including internet usage, email, and Microsoft Office applications.
  • Ability to identify operational challenges and propose effective solutions to enhance processes.
  • Access to reliable transportation for travel to care facilities and home visits.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Capable of planning, organizing, and delivering presentations effectively.

Travel Requirements:

This role involves 100% local travel to partner facilities, community hospitals, and medical practices to promote Option Care Services, process referrals, and provide live educational support.

Compensation and Benefits:

Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, along with internal equity and market alignment.

Pay Range: $38.08 - $63.45
