Position Overview:

The Clinical Transition Educator plays a vital role in delivering clinical education and training, ensuring that patients successfully transition to the Option Care Health service delivery model.

This role also involves collaboration with the sales team to enhance the number of patients transitioned to our care model.

Key Responsibilities:

  • Assess, educate, and train patients, caregivers, and facility personnel on the implementation of OCH services and products in alternative settings, whether in-home or virtually, to facilitate a smooth transition.
  • Conduct thorough patient evaluations to determine their suitability for the OCH care delivery model.
  • Collaborate with the OCH sales team to identify educational needs related to clinical transitions and develop strategies to boost patient transition rates.
  • Ensure the confidentiality of patient and proprietary information, adhering to legal standards for safeguarding sensitive data.
  • Document educational activities accurately using OCH technology and applications.

Supervisory Role:

This position does not entail supervisory responsibilities.

Essential Qualifications:

  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.

Preferred Skills:

  • Proven ability to build and maintain relationships with stakeholders at all organizational levels.
  • Knowledge of standard practices for all services offered, along with relevant industry standards (e.g., ACHC, URAC).
  • Experience in delivering high-quality customer service to both internal and external clients, with a focus on satisfaction and service standards.
  • Basic proficiency in PC applications, including internet, email, Microsoft Word, Excel, and PowerPoint.
  • Ability to identify operational challenges and propose effective solutions.
  • Access to reliable transportation for travel to various care facilities and hospitals.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational and presentation skills.

Travel Requirements:

This role requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services, process referrals, and provide educational support.

Additional Qualifications:

  • Experience in home or alternate site infusion or discharge planning is advantageous.
  • Background in expanding service provider partnerships is preferred.

Compensation:

Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, along with internal equity and market alignment.
