Job Overview

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Behavioral Health Specialist to join our team at our Mount Vernon Behavioral Health Clinic. As a key member of our interdisciplinary team, you will provide primary community support and case management services to clients of all ages, promoting their overall well-being and empowering them to achieve their goals.

Key Responsibilities
  • Conduct thorough intake screenings to assess clients' needs and develop personalized service plans.
  • Maintain regular contact with clients, including home visits, to provide advocacy and support in accessing necessary services.
  • Collaborate with community providers and agencies to ensure seamless referrals and coordination of care.
  • Document all case management activities and maintain accurate records, adhering to state and clinic requirements.
  • Participate in regular interdisciplinary team case reviews to ensure comprehensive and individualized care.
  • Embody a positive team approach, fostering a collaborative environment and welcoming new staff and visitors.
  • Maintain professional standards, adhering to treatment policies and procedures, as well as applicable federal, state, and local regulations.
Requirements
  • Associate's degree in Behavioral Health, Health Sciences, or a related field, with two years of closely related work experience.
  • Eligibility for Agency Affiliated Counselor Registration from the State Department of Health, with an active Agency Affiliated license within 90 days of hire.
  • Bilingual in English/Spanish is required.
