We are seeking a highly skilled Clinical Social Worker to join our interdisciplinary team at Matt's Place Day Treatment. As a Clinical Social Worker, you will play a vital role in providing patient-centered care to individuals living with social and behavioral struggles and autism spectrum disorders.
Key Responsibilities- Conduct psychosocial assessments to understand patient needs and develop individualized plans of care
- Collaborate with the interdisciplinary team to provide comprehensive treatment, including individual and group therapy, medication management, and recreational therapy
- Provide family and individual treatment, as indicated, to support patients and their families
- Act as an advocate for patients and families, ensuring effective communication with the care team and outside agencies
- Identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs, and coordinate with care providers and leaders to reach optimal solutions
- Master's Degree in Social Work or related field from an accredited institution
- Current State Licensure as LCSW or LMFT, or CMHC, or Psychologist
- Excellent written and verbal communication skills
- Computer skills, including email and word processing
- Experience working with patients, families, and treatment teams
- Successful completion of examination through either the National Counseling Examination (NCE) or the National Clinical Mental Health Counseling Examination (NCMHCE)
This position is located at Centerville Day Treatment and requires a full-time commitment of 40 hours per week. The hourly range for this position is $52.55, and actual hourly rate will depend on experience.
