Responsibilities- Collect and document patient data, including vital signs and medical history, in a timely and accurate manner.
- Administer medications and treatments as prescribed by physicians, ensuring patient safety and comfort.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Collaborate with healthcare providers to develop and implement individualized care plans, addressing physical, emotional, and social needs.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Graduate of an accredited practical or vocational nursing program.
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of employment.
- Ability to work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing multiple responsibilities.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
- Basic Life Support (BLS) certification, with the ability to obtain Advanced Cardiovascular Life Support (ACLS) certification as required.
- 1-2 years of experience as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in an outpatient clinic or patient care setting.
- Experience working with electronic health records (EHRs) and other healthcare technologies.
- Knowledge of evidence-based practices and quality improvement initiatives in patient care.
This position requires the ability to work in a fast-paced, dynamic environment, with frequent interruptions and changing priorities. The successful candidate will be able to work effectively in a team-based setting, prioritizing tasks and managing multiple responsibilities.
