Job Summary:

The Patient Navigator LVN coordinates access of patients through the healthcare system from initial medical diagnosis or point of referral. This role requires gathering all historical medical and insurance information for physician review during initial patient consultation. As a liaison between patients, families, outpatient oncology clinics, primary care physicians, and other support services, the Patient Navigator LVN ensures seamless communication and coordination of care. Key responsibilities include performing patient-oriented procedures and tasks within the LVN scope of practice, education, and training, as well as managing durable medical equipment (DME) supplies and performing wound care functions.

Responsibilities:
  • Coordinate patient access through the healthcare system
  • Gather historical medical and insurance information for physician review
  • Function as a liaison between patients, families, and healthcare providers
  • Perform patient-oriented procedures and tasks within the LVN scope of practice
  • Manage durable medical equipment (DME) supplies
  • Perform wound care functions
Requirements:
  • High School Diploma or GED
  • Graduate from a school for Licensed Vocational Nurse (LVN)
  • Minimum one year experience in a medical front office setting and/or a hospital-based call center
  • Minimum one year of experience working with Oncology patients preferred
Skills and Knowledge:
  • Knowledge of medical terminology
  • Ability to read, write legibly, and speak in English (and Spanish preferred) with professional quality
  • Proficiency in using computer and software programs necessary to the position
  • Ability to troubleshoot and calibrate patient care equipment
  • Excellent communication and interpersonal skills
  • Ability to work calmly and respond courteously under pressure
Licensures and Certifications:
  • Active California Licensed Vocational Nurse (LVN) license
  • Basic Life Support (BLS) certification from the American Heart Association (AHA)
