Position Overview

The Practical Nursing Specialist delivers nursing care in alignment with the established care plan for patients facing terminal illnesses, adhering to the Nurse Practice Act applicable in the relevant state.

Key Responsibilities:

  • Administers direct skilled nursing care at the bedside.
  • Assesses patient and family needs, contributing to the development and implementation of the interdisciplinary care plan.
  • Executes specific nursing tasks under the guidance of a registered nurse, including administering treatments and medications.
  • Monitors treatment effectiveness and observes patient responses.
  • Communicates significant observations or changes in patient conditions to the registered nurse, Team Manager, or Physician, ensuring accurate documentation in clinical records.
  • Ensures a safe and hygienic environment for patients and their families.
  • Maintains precise documentation, including clinical notes on interdisciplinary charts.
  • Evaluates the responses of patients and families to the provided care.
  • Utilizes a diverse range of nursing skills to implement medical orders related to medication administration, treatment execution, and monitoring of diagnostic and therapeutic measures.
  • Collaborates with the interdisciplinary team to design and implement comprehensive care plans.
  • Establishes therapeutic relationships with patients and families, encouraging their involvement in the care plan to promote independence.
  • Initiates and executes nursing protocols based on patient needs concerning physical comfort and mental well-being, including positioning, mobility, elimination, skin and wound care, personal hygiene, and communication.
  • Engages in patient and family education during daily care regarding illness understanding, treatment processes, daily living activities, self-care, and complication prevention.
  • Participates in ongoing professional development through in-service programs and community educational opportunities.
  • Contributes to educational and research initiatives as assigned.
  • Participates in the LPN/LVN on-call rotation as required to meet patient and family needs.
  • Demonstrates flexibility in scheduling, including availability for weekends and rotating shifts.

Qualifications:

  • Graduate from an accredited Practical/Vocational nursing program.
  • Current nursing license in the state where services are provided.
  • A minimum of one year of relevant nursing experience, including at least one year as an LPN/LVN in a hospital setting on an adult medical-surgical unit.
  • Equivalent experience may be considered.

Care Delivery Guidelines:

  • Does not conduct initial patient nursing assessments.
  • Operates under the supervision of a Registered Nurse.
  • Reports to the designated Team Manager.

  • Assist in lifting and transferring patients as needed.
  • Lift objects exceeding 20 pounds.
  • Stand or walk for extended periods exceeding one hour daily.

Specific vision capabilities required include:

  • Ability to adjust focus

The employee may also be required to sit, stoop, kneel, and crouch.
