Position Overview
As a Practical Nursing Specialist, you will work under the guidance of a healthcare provider or registered nurse, engaging in the assessment, planning, execution, and evaluation of patient care. Your role will involve delivering direct patient care in a manner that is both safe and effective, ultimately promoting positive health outcomes. You will be an integral member of a collaborative healthcare team.
Key Responsibilities
1. Assist licensed professionals in executing the nursing process to provide patient-centered care that is appropriate for various age groups and developmental stages.
2. Integrate cultural sensitivity into healthcare delivery, ensuring that values and language preferences are respected.
3. Utilize clinical decision-making tools to identify issues and prioritize patient care effectively.
4. Document healthcare data accurately using appropriate tools, ensuring that patient information is recorded and retrievable.
5. Educate patients and their families based on identified learning needs.
6. Plan and deliver nursing care that is safe, effective, and cost-efficient.
Collaboration and Communication
1. Coordinate with home health agencies, insurance providers, and community resources to meet patient needs.
2. Maintain ongoing communication with patients, families, and interdisciplinary staff to address and resolve potential barriers to care.
3. Facilitate outpatient care coordination and monitor patient transitions between clinic and home settings.
4. Support telehealth initiatives to address patient concerns and needs.
Professional Development
1. Engage in continuous learning to maintain and enhance nursing competencies.
2. Incorporate new knowledge into practice to achieve desired patient outcomes.
