Responsibilities

The Pharmaceutical Care Coordinator will be responsible for coordinating and managing the delivery of pharmaceutical care services in the ambulatory setting. This will include collaborating with pharmacy leadership and interdisciplinary teams to develop and implement comprehensive treatment plans, conducting D.U.E. and patient/physician education activities, and serving as a source of drug information.

To be successful in this role, you will need to have a strong understanding of clinical pharmacy practice, as well as excellent communication and interpersonal skills. A minimum of 3 years of experience in clinical pharmacy practice is required, with completion of an American Society of Health System Pharmacy PGY2 residency in ambulatory care preferred.

This is a full-time position, offering a competitive salary of $**$160,000 - $190,000 per year**, plus benefits including medical, dental, and vision coverage, as well as a generous retirement plan.
