POSITION SUMMARY:
We are actively seeking a Case Manager - Human Services Specialist IV. This full-time role offers a competitive hourly wage and is based in a dynamic environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Facilitate and coordinate client care through assessment, evaluation, planning, and execution.
  • Develop rapport and effectively communicate client requirements to various team members, ensuring follow-up.
  • Collaborate with clients, families, and both internal and external program agency personnel, serving as a primary contact.
  • Maintain comprehensive client records through clear and detailed documentation.
  • Ensure timely reporting for grants.
  • Comply with all standards and training mandated by the relevant governmental body.
  • Perform additional duties as assigned.
Requirements

Required Skills/Abilities:
  • Bilingual in Spanish is preferred, but not mandatory.
  • Proficient in data entry and grant reporting.
  • Strong organizational skills and meticulous attention to detail.
  • Ability to work autonomously; must be self-driven.
  • Excellent verbal, written, and listening communication skills.
  • Capability to understand and follow the duties, methods, and procedures required by the position.
Education and Experience:
  • High School diploma is required.
  • Bachelor's degree in Human Services, Counseling, Social Work, or a related field is required.
  • Experience in the Health and Human Services sector.
  • 2-3 years of experience working with trauma survivors from various backgrounds.
Physical Requirements:
  • Extended periods of sitting at a desk and utilizing a computer.
  • Standard working hours, with potential evening and weekend commitments.
  • Travel may be necessary.
