ID/DD Program Coordinator

- Provide initial evaluation to determine appropriateness and safety of client and environment.
- Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
- Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis.
- Participate in the development of Individual Support Plan.
- Maintain patient confidentiality.
- Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested.
- Maintain effective communication between staff, supports coordinators, and family.
- Review progress notes from paraprofessionals to determine appropriate service delivery.
- Educate paraprofessionals on individual client ISP expectations.
- Perform on site evaluation of employee performance at 90 days and annually.
- Bachelor's degree in Psychology or other social science or related field.
- Current CPR certification.
- Proof of eligibility to work in the United States.
- Current completion of child abuse clearance and Fingerprinting.
- Minimum of one year of experience with ID/DD preferred.
- Must be able to adhere to confidentiality standards and professional boundaries at all times.
- Knowledge and understanding of compliance with adherence to regulations.
- Ability to comfortably work with families with limited resources.
- Quick-thinking and astute decision making skills.
- Attention to detail.

- Must be able to travel; company does not provide vehicles or transportation.
- Frequent lifting, carrying, pushing and pulling of 25 pounds.
- Occasional lifting, carrying, pushing and pulling of 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have strong sense of smell and touch.
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
- Possible exposure to blood, bodily fluids and infectious diseases.
