  • Provide initial evaluation to determine appropriateness and safety of client and environment.
  • Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
  • Provide a written summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP to Supports Coordinators/Administrative Entities on a monthly basis.
  • Participate in the development of Individual Support Plan.
  • Maintain patient confidentiality.
  • Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested.
  • Maintain effective communication between staff, supports coordinators, and family.
  • Review progress notes from paraprofessionals to determine appropriate service delivery.
  • Educate paraprofessionals on individual client ISP expectations.
  • Perform on site evaluation of employee performance at 90 days and annually.

  • Current CPR certification.
  • Proof of eligibility to work in the United States.
  • Current completion of child abuse clearance and Fingerprinting.
