Responsibilities

Hospice RN Full-Time On Call 7on/7off Monday 5p - Monday 8a

  • Coordinate patient care effectively,
  • Ensure continuous assessment of each patient's and family's needs
  • Implement the interdisciplinary care plan.
  • Deliver services in accordance with physician's orders and the established plan of care (POC).
  • Ensure compliance with federal and state regulations in care delivery.
Qualifications
  • Must be a graduate of an accredited nursing program.
  • Must hold a valid nursing license in the state of practice.
  • Must have a minimum of one year of experience as a licensed registered nurse (RN).
  • Basic technology skills, including the use of mobile devices, are required.

Preferred Education and Experience

  • Experience in Medicare home health or hospice is preferred.

Requirements

  • Must possess a valid state driver's license.
  • Must maintain automobile liability insurance as mandated by law.
  • Must have reliable transportation in good working order.
  • Must be capable of safely driving in various weather conditions.
  • Must hold CPR certification for healthcare providers.
