About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager at Pinnacle Home Hospice Services. As a Hospice Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Manage a caseload of hospice patients and provide comprehensive care, including assessments, care planning, and coordination of services.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure seamless care delivery.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient and their family.
  • Provide education and support to patients and their families on end-of-life care, pain management, and other relevant topics.
  • Conduct regular home visits to assess patient needs, provide care, and monitor progress.
  • Document patient information, progress, and outcomes in a timely and accurate manner.
Requirements
  • Current state RN license.
  • At least one year of experience working with adult and geriatric medical/surgical patients in a hospital, SNF, or home health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a high level of autonomy and flexibility.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
