Responsibilities

Hospice RN - Full Time On Call 7on/7off Monday 5p - Monday 8a

  • Coordinate patient care effectively,
  • Conduct continuous assessments of each patient and family’s needs
  • Implement the interdisciplinary care plan.
  • Deliver services in accordance with physician orders and the established plan of care (POC).
  • Ensure compliance with federal and state regulations in care delivery.
Qualifications
  • Graduate of an accredited nursing program.
  • Current nursing license in the state of practice.
  • A minimum of one year of experience as a licensed registered nurse (RN).
  • Basic technology skills, including proficiency with mobile devices.

Preferred Education and Experience

  • Experience in Medicare home health or hospice is preferred.

Requirements

  • Valid state driver’s license
  • Maintain automobile liability insurance as required by law
  • Dependable transportation in good working condition
  • Ability to drive safely in various weather conditions
  • Current CPR certification for healthcare providers
