Job Title: Certified Diabetes Care Education Specialist

Sea Mar Community Health Centers is seeking a Certified Diabetes Care Education Specialist to join our team. As a Certified Diabetes Care Education Specialist, you will play a critical role in delivering comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and endocrinology care.

Job Summary:

The Certified Diabetes Care Education Specialist will provide collaborative, comprehensive, and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. The ideal candidate will possess a strong understanding of diabetes care and education, as well as excellent communication and interpersonal skills.

Responsibilities:
  • Maintain complete and up-to-date charts and statistical program records.
  • Establish and maintain professional working relationships with team members and other Sea Mar staff.
  • Assist with clerical and statistical program work as needed.
  • Attend selected clinic, MSS, PHS Department, and WIC meetings and participate in continuing education as needed.
  • Act as a preceptor for dietetic interns, as appropriate.
Requirements:
  • Possess a Bachelor's or Master's degree in nutrition from an accredited university.
  • Completed an accredited dietetic internship program or Coordinated Undergraduate Program in Dietetics.
  • 7 hours of HIV/AIDS course.
  • WA State Registration and Certification for Dietitian or RD/CD eligible with attainment of these credentials within time period stated in the Sea Mar Nutrition Program Policies and Procedures Manual.
  • Medical Assistant-Registered Certification required for Hemoglobin testing for WIC RD.
