Job Summary: We are seeking a highly skilled Clinical Pharmacist to join our team at PacMed Clinics DBA Pacific Medical Centers. As a Clinical Pharmacist - Ambulatory Care Specialist, you will play a critical role in improving the health of our patients by providing collaborative drug therapy management services, assisting medical personnel and patients with drug information and identification, and developing and sustaining productive patient relationships.

Key Responsibilities:

* Collaborate with primary care providers and staff to improve patient health
* Provide collaborative drug therapy management services
* Assist medical personnel and patients with drug information and identification
* Develop and sustain productive patient relationships
* Evaluate and resolve potential drug therapy problems
* Interview patients to obtain information regarding drug use and document the information in the patient's medical record
* Confer with medical personnel regarding care and treatment of patient-related clinical diagnosis, drug combinations, and dosage forms
* Provide direction to pharmacy interns, residents, fellows, pharmacy technicians, and other pharmacy personnel

Requirements:

* PharmD graduate of an accredited college of pharmacy or equivalent education/experience
* Washington Pharmacist License
* 2 years of ambulatory pharmacy experience

Preferred Qualifications:

* Completion of a PGY1 or PGY2 residency or equivalent education/experience
* Certified Diabetes Educator (or willing to obtain) or Hypertension Clinician Certification (or willing to obtain)
