Job Title: Certified Diabetes Care Education Specialist

SeaMar Community Health Centers is seeking a Certified Diabetes Care Education Specialist to join our team. As a Certified Diabetes Care Education Specialist, you will play a vital role in providing comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and endocrinology care.

Responsibilities:
  • Deliver comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and endocrinology care.
  • Provide collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan.
  • Support and advocate for people affected by diabetes to optimize quality care.
  • Promote self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.
  • Maintain complete and up-to-date charts (documenting counseling session as specified by program guidelines in legible form), and statistical program records.
  • Establish and maintain professional working relationships with team members and other SeaMar staff.
  • Assist with the clerical and statistical program work as needed.
  • Attend selected clinic, MSS, PHS Department and WIC meetings and participate in continuing education as needed.
  • Act as a preceptor for dietetic interns, as appropriate.
Requirements:
  • Possess a Bachelor's or Master's degree in nutrition from an accredited university.
  • Completed an accredited dietetic internship program or Coordinated Undergraduate Program in Dietetics.
  • 7 hours of HIV/AIDS course.
  • WA State Registration and Certification for Dietitian or RD/CD eligible with attainment of these credentials within time period stated in the SeaMar Nutrition Program Policies and Procedures Manual.
  • Medical Assistant-Registered Certification required for Hemoglobin testing for WIC RD.
