Compensation:
$77.72 Hourly

Your Responsibilities:
  • Deliver comprehensive advanced nursing care, encompassing physical assessments, history-taking, urgent interventions, and chronic condition management, while adhering to established care standards.
  • Collaborate with patients, families, and healthcare teams to evaluate, plan, and oversee care across diverse settings, including in-home evaluations and monitoring health status changes.
  • Ensure compliance with regulatory standards and quality metrics.
  • Educate patients and caregivers on chronic disease management, self-care practices, and appropriate responses to health changes, while providing support to interdisciplinary team members.
  • Manage medical documentation, billing information, and engage in committees and advisory groups focused on quality enhancement and professional growth.

Qualifications:
  • Master's degree in Nursing or another graduate degree from a nurse practitioner program recognized by the New York State Education Department for NP certification (licensure) is required.
  • Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner is mandatory.
  • A minimum of two years of experience as a nurse practitioner utilizing the full scope of practice is preferred.
  • Experience in clinical home care or two years of managerial experience is preferred.
  • Demonstrated understanding of HEDIS and QARR quality measures, ICD-10, and CPT coding for service reimbursement is required.
  • Bilingual skills may be necessary based on operational needs.
