At VNS Health, our Clinical Practice Team Leads leverage their Nurse Practitioner expertise to transform the landscape of patient-centered care for individuals in New York. These skilled professionals deliver both primary and palliative care throughout various stages of a patient's life, conducting home visits for acute, annual, urgent, and end-of-life care, as well as virtual consultations to effectively manage chronic illnesses.

Compensation:
$77.72 Hourly

Your Responsibilities:
- Deliver comprehensive advanced nursing care, encompassing physical assessments, history-taking, urgent interventions, and chronic condition management, while adhering to established care standards.
- Collaborate with patients, families, and healthcare teams to evaluate, plan, and oversee care across diverse settings, including in-home evaluations and monitoring health status changes.
- Ensure compliance with regulatory standards and quality metrics.
- Educate patients and caregivers on chronic disease management, self-care practices, and appropriate responses to health changes, while providing support to interdisciplinary team members.
- Manage medical documentation, billing information, and engage in committees and advisory groups focused on quality enhancement and professional growth.
Qualifications:
- Master's degree in Nursing or another graduate degree from a nurse practitioner program recognized by the New York State Education Department for NP certification (licensure) is required.
- Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner is mandatory.
- A minimum of two years of experience as a nurse practitioner utilizing the full scope of practice is preferred.
- Experience in clinical home care or two years of managerial experience is preferred.
- Demonstrated understanding of HEDIS and QARR quality measures, ICD-10, and CPT coding for service reimbursement is required.
- Bilingual skills may be necessary based on operational needs.
