Job Summary

We are seeking a skilled Ambulatory Care Coordinator to join our team at The Brooklyn Hospital Center. In this role, you will play a vital part in improving preventive and chronic care outcomes for patients.

The successful candidate will work closely with patients, families, and healthcare teams to develop personalized care plans that address each patient's unique needs. Using registry information, you will identify patients with clinically significant chronic conditions and other prevalent health issues within our community.

Key Responsibilities
  • Care Coordination: Coordinate care services to improve patient outcomes.
  • Patient-Centered Care: Work collaboratively with patients and families to plan and implement individualized care plans.
  • Registry Management: Utilize registry information to identify patients requiring proactive care management.

In addition to these core responsibilities, the ideal candidate will possess strong communication and team-building skills, with the ability to effectively advocate for patients and collaborate with healthcare professionals.

Requirements
  • Bachelor's Degree in a healthcare-related field.
  • 2+ years of experience in care coordination and/or ambulatory healthcare settings.
  • Strong experience with quality management activities and outcome measures.
  • Excellent communication, teamwork, and advocacy skills.
  • Demonstrated proficiency in Microsoft Office applications (Word, Excel, PowerPoint).
  • Bilingualism in Spanish/English is preferred.
