We are seeking a highly skilled and compassionate Supervising Professional to join our team at Accra Care. As a key member of our team, you will be responsible for setting up and managing direct care services for participants who qualify for direct care services as determined by their assessment.

Key Responsibilities
  • Develop participant service plans with input from the Participant's Representative (PR) and assessment document.
  • Assist with care delivery competency trainings as needed.
  • Provide resources for participants, PRs, or workers on appropriate program use and other services to meet health and safety needs.
  • Assist in training, supervision, and evaluation of services, ensuring quality assurance of direct care services.
  • Stay up-to-date on current policies and changes to the program, participating in committees and meetings to enhance knowledge of services beneficial to clients.
Requirements
  • Supervising Professional must be qualified in at least one of the following ways:
  • A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.
  • An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.
  • Willingness to be trained on services needed by people with disabilities and the elderly in Minnesota.
  • Willingness to be trained on home health care requirements, regulations, and protocols.
  • Demonstrated creativity, critical thinking, and problem-solving skills.
  • Able to affect change and demonstrate positive leadership.
  • High commitment to service and professionalism.
  • Excellent organizational, problem-solving, communication, and interpersonal skills.
  • Willingness to be trained on federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota.
  • Must possess intellectual strength and have a keen understanding of internal systems needed to support services.
  • Must pass a background study with the Minnesota Department of Human Services.
  • Have a valid driver's license, clean driving record, and reliable transportation for work driving purposes.
