About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Care LPN Visit Nurse. As an LPN, you will play a critical role in providing high-quality clinical care and services to our clients in the comfort of their own homes.

As a Home Care LPN Visit Nurse, you will be responsible for assessing the needs of our clients, developing and implementing individualized care plans, and providing ongoing support and education to clients and their families. You will also work collaboratively with our interdisciplinary team to ensure that our clients receive comprehensive and coordinated care.

We are looking for an LPN who is passionate about delivering exceptional patient care, has excellent communication and interpersonal skills, and is able to work independently in a fast-paced environment. Responsibilities
  • Assess the needs of clients and develop individualized care plans
  • Provide ongoing support and education to clients and their families
  • Work collaboratively with the interdisciplinary team to ensure comprehensive and coordinated care
  • Administer medications and treatments as ordered by the physician
  • Monitor and report any changes in client condition to the physician and other members of the healthcare team
Qualifications
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Minnesota
  • Experience in home care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently in a fast-paced environment
  • Ability to lift 25 pounds and stand for long periods of time
