Exciting Opportunity: Case Management Specialist at Reach for Resources

Position Overview:

  • Deliver case management services for adults or minors eligible for the CADI or BI Waiver.
  • Assist clients in accessing supportive services that cater to their social, recreational, health, educational, and vocational needs.
  • Oversee a caseload of up to 45 clients.

Primary Duties:

  • Provide case management services in accordance with DHS and county regulations.
  • Conduct regular meetings with clients and facilitate necessary referrals.
  • Ensure confidentiality and maintain precise documentation.
  • Offer community-based services centered around individual needs.
  • Engage in public relations, networking, and liaison activities.
  • Effectively utilize available resources to identify appropriate services for clients.
  • Collaborate with colleagues and participate in agency activities as needed.

Position Scope:

The case management specialist will work closely with individuals, families, and support teams to deliver essential mental health, social, educational, and vocational services. Responsibilities include assessing client needs, developing tailored plans, connecting to community resources, and monitoring service delivery.

Qualifications:

  • Bachelor's degree in social work, psychology, sociology, or a related field.
  • Experience in supporting individuals with disabilities or mental health challenges.
  • Ability to work autonomously, manage stress effectively, and pass a background check.
  • Valid driver's license, dependable transportation, and comprehensive auto insurance.
