We are seeking experienced and caring LPNs to join our team at Home Health Care Incorporated. Successful applicants will have a proven track record as an LPN, with home care experience a huge benefit but not required.
Responsibilities- Provide patient care according to client's care plan and needs.
- Perform wound care treatments and assist clients with medication management.
- Teach the client and/or family members about care and support skills.
- Communicate regularly with RN Case Managers, Supervisors, and the Care Team.
- Licensed as a Practical Nurse / LPN in the state of Minnesota.
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
- Demonstrated ability to work autonomously.
- Prior Nursing experience in a TCU, Hospice, or Home Health setting.
- Experience with Electronic Medical Records (EMR) software is preferred.
