Job Overview

The Transportation Services Performance Analyst will oversee the effectiveness and engagement of transportation services within the Social Care Network, specifically focusing on the Southern Tier region of New York. This role serves as a key resource for participating organizations, ensuring optimal performance and adherence to best practices in transportation services. Reporting directly to the Network Operations Manager, this position plays a crucial role in stakeholder engagement and operational excellence.

Key Responsibilities:
  • Act as a regional authority on the 1115 waiver health-related social need transportation enhanced services, ensuring timely delivery of eligible services throughout the Southern Tier and providing actionable insights to enhance service delivery across community-based organizations (CBOs).
  • Develop and disseminate training materials to facilitate the hiring and onboarding processes for personnel involved in HRSN transportation services.
  • Utilize the Care Compass Unite Us system effectively to support transportation service delivery, while suggesting enhancements to improve system functionality.
  • Collaborate with organizations using integrated systems to ensure accurate workflow documentation and data entry.
  • Maintain comprehensive knowledge of New York State Department of Health HRSN transportation regulations, assisting in the creation of communications to keep network participants informed of compliance requirements.
  • Support local CBOs in enhancing their public and private transportation capabilities by leveraging both local and national expertise to share effective practices.
  • Regularly engage with network staff to gather feedback on challenges faced in HRSN transportation services, and work collaboratively to devise actionable solutions.
  • Provide customized support to each CBO to ensure they meet and sustain readiness criteria, including validating completion of necessary training by staff.
  • Integrate existing transportation services with the new 1115 waiver housing enhanced services to create a cohesive support system.
  • Collaborate with the Network Operations Manager to formulate performance improvement plans (PIPs) for CBOs, offering additional training and technical assistance as needed.
  • Monitor and manage the performance of social care network transportation services, conducting monthly reviews of performance metrics and addressing any identified gaps in service delivery.
  • Generate performance reports (scorecards) for organizations providing HRSN transportation services, utilizing data analysis to highlight areas for improvement and influence necessary workflow adjustments.
  • Foster a culture of continuous improvement in program design, execution, and monitoring.
Qualifications:
  • Education: A Bachelor’s degree is required, along with 3-5 years of project management experience.
  • Experience in screening, navigation, or case management, particularly with low-income or Medicaid populations.
  • Ability to thrive in a fast-paced environment and adapt to the evolving nature of new programs.
  • Strong critical thinking skills to address workflow challenges and manage program performance effectively.
  • Proficient in conveying complex information clearly and interpreting intricate data from various sources.
  • Client-focused and results-oriented.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and capable of facilitating meetings in-person or via virtual platforms.
Work Environment:

This position operates under a hybrid work model, allowing for remote work or office-based responsibilities in a professional setting. Some travel may be necessary for meetings with partners or network stakeholders.
