Job Summary

As a key member of the Population Health Service Organization's Community Care Coordination team, the Community Health Worker will play a vital role in bridging the gap between healthcare providers and underserved populations. This hybrid role will involve working in the community, at physician/provider clinics, and virtually, to assist patients in understanding their care plans and instructions, addressing social determinants of health, closing care gaps, and coaching patients to effectively manage their health conditions and advance self-care.

Key Responsibilities
  • Connect uninsured and under-insured populations with community health and social service resources and benefits
  • Identify and address medical, mental health, and dental needs, and link patients with providers
  • Complete eligibility applications and address barriers to access
  • Provide follow-up and support to ensure patients receive the necessary resources and care
Requirements
  • Associate's or Bachelor's degree in Health Services, Public Health, Social Work, Psychology, or related field
  • Certified as a Community Health Worker, or obtain certification within 12 months of hire
  • Minimum of three years of experience in a community resource setting, or four years without a degree
  • Effective oral and written communication skills
  • Ability to educate staff and patients on the healthcare/social service system
  • Proficiency in communication and computer technologies
  • Highly organized with ability to keep accurate notes and records
  • Ability to work collaboratively with healthcare providers and staff in a fast-paced, patient-service-oriented environment
Preferred Qualifications
  • Bilingual (ability to speak a relevant second language)
