About the Role

We are seeking a highly skilled and compassionate Licensed Master Social Worker (LMSW) to join our team as a Home Health Social Worker. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and support to individuals in their homes.

Responsibilities
  • Conduct in-home patient visits to assess and address their social and emotional needs.
  • Collaborate with healthcare providers, family members, and community resources to develop and implement personalized care plans.
  • Provide education and support to patients and their families on managing chronic conditions, accessing community resources, and navigating the healthcare system.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
Qualifications
  • Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
  • Current licensure as a Licensed Master Social Worker (LMSW) in the state of employment.
  • At least one year of experience in a healthcare setting, preferably in home health or hospice care.
