Job Description:

As a Social Worker II with the Department of State Health Services, you will provide advanced and comprehensive case management consultation for families with children who have health risks, conditions, or special health care needs. You will work under the supervision of the Regional Program Manager to provide consultation, assessment, and service planning for families enrolled in or applying for various programs, including Children with Special Health Care Needs (CSHCN), Supplemental Security Income (SSI), and others.

Key Responsibilities:
  • Conduct home visits to determine eligibility, complete family needs assessments, and develop service plans.
  • Assist families with the eligibility determination process for CSHCN.
  • Connect families to community resources and provide information and referral services.
  • Participate in community events and provide training, consultation, and technical assistance to program contractors and providers.
  • Work under minimum supervision with considerable latitude for independent judgment.
Requirements:
  • Current license to practice Social Work in the State of Texas.
  • Knowledge of case management principles and practices, as well as children's programs and services.
  • Ability to work effectively as a team member in an interdisciplinary setting.
  • Ability to interpret and apply program policies and procedures.
  • Ability to maintain confidentiality and provide professional leadership.
