Job Summary

As a Community Health Navigator at Memorial Hermann Health System, you will play a vital role in connecting uninsured and underinsured populations with community health and social service resources and benefits. Your goal will be to identify and address barriers to access, ensuring that these individuals can benefit from and become knowledgeable about the best community resources for their unique needs.

Key Responsibilities
  • Obtain demographic, insurance, and financial information, as well as health and social needs from patients.
  • Communicate health information, make referrals to health and social service providers, and schedule appointments.
  • Advocate on behalf of patients and families, entering required information into the computer system in an accurate and timely manner.
  • Coach patients and families on accessing community resources and explain eligibility applications.
  • Provide complex counseling, identifying alternative resources for patient care, and educate families on the role and expectations of medical and dental homes.
  • Monitor, review, and resolve patient needs, following up after visits to determine effectiveness and follow-through of referrals.
Requirements
  • High school diploma or GED
  • Certification as a Community Health Worker preferred
  • Two years of experience working in a community or healthcare setting
  • Advanced computer skills and bilingual ability
  • Effective oral and written communication skills
