Job Summary

Sutter Health is seeking a skilled Licensed Vocational Nurse (LVN) to join our Home Health team. As an LVN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities
  • Provide nursing care to patients in their homes, under the supervision of a registered nurse or physician.
  • Assess patient needs, develop and implement individualized care plans, and evaluate patient progress.
  • Administer medications, perform wound care, and provide education to patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Document patient information accurately and thoroughly.
Requirements
  • Graduate of an accredited school of vocational nursing.
  • Current LVN license in California (or LPN license in Oregon).
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
  • 1 year of recent relevant experience in home health or a related field.
