Care Coordinator Job Description

At North Homes, Inc., we are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring seamless and top-notch care for our clients and their families.

Key Responsibilities:
  • Assessing Client Needs: Conduct thorough intake and preliminary screening processes to identify client needs and develop personalized care plans.
  • Connecting Clients with Resources: Facilitate connections with internal and external services and resources to meet client needs.
  • Ensuring Continuity of Care: Coordinate with treatment teams to ensure continuity of care and effective communication.
  • Developing Integrated Treatment Plans: Collaborate with internal and external providers to develop person-centered and family-centered integrated treatment plans.
  • Assessing and Addressing Social Determinants of Health: Identify and address Social Determinants of Health (SDOH) to promote holistic care.
  • Collaborating with Referral Resources: Work closely with internal and external referral resources to ensure timely and effective referrals.
  • Documenting and Maintaining Accurate Records: Document and maintain accurate and timely care coordination summaries and client records.
Requirements:
  • Education: High School Graduate or GED
  • Background Study: Must pass a DHS background study and meet applicable requirements and regulations.
  • Age and Driver's License: Must be at least 21 years old and have a valid Minnesota Driver's License.
Preferred Experience:
  • Behavioral Health Experience: Experience and knowledge in behavioral health programming.
  • Education and Experience: 2 years of college in a healthcare or human service related field or 3 years of experience preferably working within a behavioral health clinic or social services.
Skills and Abilities:
  • Client-Centered Care: Excellent client-centered care, interpersonal, and organizational skills.
  • Communication: Excellent oral and written communication skills.
  • Teamwork: Ability to be a team player and collaborate with internal and external providers and agencies.
  • Adaptability: Ability to adapt and deal with challenging situations and changes.
  • EMR System: Ability to use Electronic Medical Record (EMR) system.
  • Confidentiality: Maintain confidentiality and be non-judgmental in regards to lifestyle, race, and/or economic status.
