Position Objective:

The PATH (Projects for Assistance in Transition from Homelessness) Case Manager is tasked with assisting individuals experiencing homelessness within the PATH initiative to secure stable housing and access necessary benefits. This role involves conducting thorough assessments, formulating personalized service plans, and tracking client progress. The case manager will engage in weekly community-based case management to support clients in achieving and sustaining housing and benefits while enhancing their self-determination through effective illness management. Collaboration with community organizations and homeless service providers, as well as mental health service teams, is essential. Participation in weekly client discussions, monthly program management meetings, and performance monitoring is also required.

Qualifications:

  • Bachelor's Degree in a health-related discipline, with a preference for two years of experience in mental health
  • Possession of a valid driver's license and dependable transportation

Key Skills:

  • Proven education, training, or experience in mental health services and/or co-occurring disorders
  • Strong written and computer skills for timely documentation
  • Excellent organizational abilities and computer proficiency
  • Self-motivated with the capacity to work independently, collaboratively, and within community settings
  • Expertise in individual, family, and group counseling, as well as case management
  • Ability to educate and support clients, families, staff, or community resources
  • Maintain open and effective communication with the team
  • Ability to perform or learn de-escalation techniques
  • Professional collaboration with community agencies
  • Comfortable working independently in community settings and client residences
  • Ability to establish a healthy work-life balance
  • Challenge preconceived notions regarding mental illness.

Core Responsibilities:

  • Deliver mental health rehabilitation services and support to adults and families to reduce hospitalizations and crisis situations while enhancing community functioning to meet rehabilitative, resilience, and recovery goals.
  • Ensure continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, adhering to applicable regulatory and accreditation standards.
  • Act as a team member in providing mental health services to clients and their families, integrating recovery issues into their daily lives.
  • Offer case management services, crisis intervention, skills training, outreach, and group services.
  • Complete documentation within required timeframes
  • Participate in meetings
  • Contribute to quality improvement initiatives
  • Fulfill additional duties necessary to address the needs of the seriously mentally ill and homeless populations.

Employment Type: Full-time
