Care Coordination Services

Catholic Charities of the Diocese of Albany is seeking a skilled Nurse Practitioner/Physician Assistant to join our team in providing high-quality care to individuals seeking Medication Assisted Treatment.

Key Responsibilities:
  • Conduct medical assessments of individuals seeking Medication Assisted Treatment
  • Determine availability for immediate provision of Medication Assisted Treatment
  • Provide Medication Assisted Treatment and additional medications as deemed appropriate
  • Work with patients to overcome barriers to treatment, including pharmacy access
  • Develop and implement service plans with the Bridge Clinic Coordinator and agency staff
  • Participate in treatment team meetings to review and update service planning
  • Facilitate referrals to community-based Medication Assisted Treatment Providers for maintenance
  • Consult with patients assigned to maintenance providers to identify barriers to successful transition
  • Complete patient assessments and documentation through agency-issued medical records systems
  • Complete ongoing notation of additional services provided
Requirements:
  • NP, PA, or MD licensure
  • Current DEA licensure
  • Current Buprenorphine DATA waiver
  • Current provider card in Basic Life Support (BLS)
  • Active Liability/Malpractice Insurance
