At Computer Aid, we are seeking a skilled Clinical Care Coordinator to join our team in Delaware. As a key member of our care coordination team, you will be responsible for providing community-based care management services to our members in the state of Delaware.

About the Role

This is a full-time hybrid position that requires working flexible hours to meet member needs. The ideal candidate will have prior experience in case management and discharge planning, with a focus on geriatric special needs, behavioral health, and home health. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with healthcare providers, family members, and caregivers.

Key Responsibilities
  • Travel to members' homes, nursing facilities, and other community-based settings to conduct face-to-face assessments and provide care coordination services.
  • Assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex healthcare needs.
  • Collaborate with healthcare providers, family members, and caregivers to develop and implement personalized care plans.
  • Provide education and resources to members and their families to help them understand their healthcare options and make informed decisions.
  • Facilitate transitions to alternate care settings, such as hospital to home or nursing facility to community setting, using an integrated care team approach.
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse license in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning, with a focus on geriatric special needs, behavioral health, and home health.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in electronic documentation systems and Microsoft Office applications.
  • Reliable transportation and a valid driver's license.
Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus benefits including medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
