Job Title: Delaware Care Coordinator Professional

About the Job:

We are seeking a skilled Care Coordinator to join our team at Computer Aid. The successful candidate will work with members in the state of Delaware, providing community-based care coordination and management services.

The ideal candidate will have 5+ years of experience in case management and discharge planning, including experience working with complex health care needs. They will be responsible for conducting face-to-face assessments, developing service plans, and coordinating care across various settings.

Key Responsibilities:

* Conduct regular home visits, assessments, and telephonic contacts with members to identify their health care needs and develop personalized service plans.
* Collaborate with healthcare providers, caregivers, and other stakeholders to ensure seamless care coordination and delivery.
* Develop and implement comprehensive care plans that address members' physical, behavioral, and social needs.
* Facilitate transitions between care settings, such as hospital to home or nursing facility to community setting.

Requirements:

* A current Registered Nurse license in the state of Delaware is required.
* At least 5 years of experience in case management and discharge planning, preferably in a geriatric or behavioral health setting.
* Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
* Ability to travel within assigned territory and maintain accurate records in electronic documentation systems.

Preferred Qualifications:

* Certified Case Manager (CCM) or Licensed Social Worker (LBSW/LMSW/LCSW) certification.
* Experience working with HIV/AIDS, behavioral health, or developmental disabilities populations.
* Knowledge of Medicare and Medicaid programs and regulations.
