CAI is a leading healthcare provider seeking an experienced Care Coordination Specialist to join our team in Delaware.

About the Role:

We are looking for a skilled and compassionate professional to provide community-based care coordination services to our clients. As a Care Coordination Specialist, you will work closely with healthcare teams and community resources to ensure our members receive optimal services in the least restrictive environments.

The ideal candidate will have a current RN license in Delaware and extensive experience in case management and discharge planning. A strong understanding of healthcare coordination, patient assessment, and chronic care management is essential for this role.

Key Responsibilities:
  • Travel to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments and subsequent telephonic contact.
  • Assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex healthcare needs.
  • Collaborate with healthcare teams, including PCPs, occupational therapists, behavioral health professionals, and providers, to ensure seamless care transitions.
  • Develop and implement individualized care plans that address members' physical, behavioral, social, and psychosocial needs.
  • Educate members and caregivers on healthcare needs, available benefits, and resources, including options for long-term care delivery.
  • Provide ongoing support and education to help members achieve their goals and overcome obstacles to optimal care.
Requirements:
  • Current Registered Nurse license in Delaware
  • 2+ years of experience in case management and discharge planning, including facility setting discharges
  • Experience completing assessments, developing service plans, and coordinating care across the continuum
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Ability to travel throughout Delaware (may require 2-3 trips per week)
Salary and Benefits:

The estimated annual salary for this role is $85,000 - $110,000, depending on experience. CAI offers a comprehensive benefits package, including medical, dental, and vision coverage, retirement savings, and paid time off.
