Job Summary:
Clinical Care Coordinators play a vital role in providing community-based care coordination and management services for clients. This full-time, hybrid position will be based in Delaware, requiring the ability to travel within the state.

We are seeking a motivated Clinical Care Coordinator to join our team. The ideal candidate will have prior experience with coordinating, implementing, and evaluating effective treatment plans. A strong background in case management and discharge planning is essential.

The successful candidate will provide community-based care coordination and management services for our clients in Delaware. Responsibilities include traveling to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments and subsequent telephonic contact. Additionally, the Clinical Care Coordinator will assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex health care needs.

Key responsibilities include:

  • Assessing, planning, coordinating, implementing, and evaluating care for eligible members with chronic and complex health care needs.
  • Coordinating care across the continuum of services and assisting members with physical, behavioral, long-term services and supports (LTSS), social, and psychosocial needs.

Requirements:
To be considered for this position, candidates must possess a current Registered Nurse license in the state of Delaware and at least 2 years of experience in case management and discharge planning. Additional requirements include experience completing assessments, developing service plans, and collaborating with PCPs, occupational therapists, and behavioral health professionals. Proficiency in PC-based word processing and database documentation is also necessary. Reliable transportation and the ability to meet regulatory deadlines are required.

Benefits:
