Job Overview

We are seeking a skilled Care Coordinator to join our team at CAI in Delaware. As a Care Coordinator, you will play a vital role in providing community-based care coordination and management for clients with complex needs.

About the Role

This is a full-time position that requires collaboration with healthcare teams to ensure clients receive appropriate services. Your key responsibilities will include assessing, planning, and implementing care plans, facilitating transitions between care settings, and educating clients about available resources.

Key Responsibilities
  • Care Coordination: Assess, plan, and implement care plans for clients with chronic and complex needs.
  • Transitions: Facilitate smooth transitions between care settings, ensuring continuity of care.
  • Education: Educate clients and caregivers about available resources, health education, and client services.
  • Collaboration: Work closely with interdisciplinary teams to coordinate care and services.
Requirements
  • Experience: 2+ years of experience in case management and discharge planning, preferably in a geriatric or home health setting.
  • Licensure: Current RN licensure in the state of Delaware.
  • Skills: Strong assessment, planning, and implementation skills; excellent communication and interpersonal skills; ability to work independently and as part of an interdisciplinary team.
Salary and Benefits

The estimated salary for this position is $$83,000 - $110,000 per year, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
