Overview

The Licensed Practical Nurse (LPN) Intake Coordinator plays a crucial role in managing and assessing incoming referral and order communications.

Key Responsibilities

  • Handles incoming calls and effectively directs them to the appropriate Intake Team members.
  • Communicates essential patient and referral details to the relevant Intake personnel and branches.
  • Collects demographic and clinical data from clients, healthcare providers, and other referral sources to compile comprehensive referrals.
  • Facilitates the transfer of referral information to the Intake Specialist RN.
  • Keeps a detailed log of all incoming communications.
  • Ensures signed referrals are sent to the correct county/parish offices, DME, private duty, etc.
  • Confirms the licensure of new referring physicians for referrals processed in the department.
  • Investigates coverage or payment sources for each referral, verifying commercial policies and HIQA.
  • Secures authorization for visits from insurance or Medicaid for EPSDT patients as necessary, and keeps the insurance case manager informed about patient status and the need for additional visits during the care episode.
  • Informs prospective patients, families, healthcare providers, and hospitals about available home health services.
  • Acts as a communication bridge between clinical and marketing teams.
  • Participates in annual in-service training sessions.
  • Manages patient ping feed throughout the day in relevant markets.
  • Distributes patient ping data to the appropriate Patient Care Manager, field clinicians, or sales representatives in relevant markets.
  • Performs any additional duties as assigned.

Qualifications

Licensure Requirements

  • Active LPN license in the state of practice.
  • Valid Driver's License, vehicle insurance, and access to reliable transportation or public transit.
  • CPR Certification is mandatory.
  • At least one (1) year of experience in medical office procedures or intake processes.
