Job Summary

Excelsior Wellness is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in supporting families and youth in achieving their therapeutic goals.

Key Responsibilities
  • Develop and implement community resource programs to achieve desired outcomes for students.
  • Understand and actively promote WISe programs throughout the county, identifying ways in which efforts aimed at improving outcomes for students can be coordinated.
  • Recognize barriers that occur in a system of care and develop and implement practices necessary to sustain system of care improvements.
  • Assess opportunities and barriers to achieve WISe services across all systems.
  • Develop long and short-term goals and objectives for the development and implementation of WISe program services across all systems of care.
  • Seek opportunities to effectively communicate with the public and private child and family serving agencies to share information about the program.
  • Serve as liaison between all child and family serving agencies and the WISe treatment team.
  • Coordinate cross-agency communication and planning issues as needed.
  • Provide transportation when taking students/families to activities, shopping centers, school, court, family visits, etc.
Requirements
  • High School Diploma required, BA in counseling or related field preferred.
  • Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
  • Comprehensive knowledge of community resources and public systems.
  • Minimum three years of working with youth and families.
  • Exceptional communication and interpersonal skills.
  • Community Health Worker certification preferred.
  • Must have strong multitasking and organizational skills.
  • Ability to work in a fast-paced environment and make ethical and safe decisions.
  • Ability to manage individual stress and perform essential job functions in a safe and professional manner.
  • Must be at minimum 21 years of age.
  • Ability to pass criminal background check, per organizational standards.
  • Valid driver's license and acceptable driving record, per organizational standards.
