Responsibilities
  • Accountability for the achievement of optimal outcomes within the standards of practice.
  • Collaborates by working with patients, families, caregivers, and the health care team locally and within the Shriners Children's system to jointly communicate, problem solve, and share accountability for optimal outcomes.
  • Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team, including the patient's Primary Care/Medical Home by coordinating resources and services necessary.
  • Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
  • Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
Qualifications
  • Required: Bachelor in Nursing, RN licensure in WA, 3 years experience in nursing, Case Management Certification (CMC, ACM or ANCC), if not currently certified - certification will be required within 12 months of hire/eligibility, COVID-19 Vaccination.
  • Preferred: Master's in Nursing, 3-5 years Care Coordination/Case Management experience, 5+ years of clinical experience in Pediatrics, Burn care and/or Orthopaedics, Experience with utilization review/management with a working knowledge of InterQual criteria.
