Job Title: Clinical Care Coordinator

Job Summary:

We are seeking a Clinical Care Coordinator to join our team. The successful candidate will provide clinical services to patients admitted to our Evaluation and Treatment (E&T) inpatient units. The coordinator will coordinate and plan patient discharge, connecting clients to various resources such as housing and community services.

Key Responsibilities:
  • Provide case management services, including coordination of housing, Medicaid enrollment, insurance enrollment, and connection with other community resources.
  • Provide 1:1 counseling services to clients as needed.
  • Facilitate psychoeducational groups as a component in the therapeutic treatment plan.
  • Provide outreach and follow-up to individuals discharged from the Evaluation and Treatment Facilities, as appropriate.
  • Coordinate with various mental health and chemical dependency treatment agencies, including DCR's, psychiatric and medical hospitals, and the criminal justice system.
Requirements:
  • Master's degree in the behavioral sciences.
  • Active BLS within 90 days of hire and must maintain active BLS.
  • Must have reliable transportation, insurance, and an active driver's license and be willing to use personal vehicle in the course of work.
Preferred Qualifications:
  • At least one year of related experience working with psychiatrically impaired populations.
Work Environment:

The work environment is generally performed in an office setting, which may involve sitting and use of computers for four to eight hours per day. May be required to lift pounds. Blood-borne Pathogen job classification - occupational exposure to bloodborne pathogens.

Pay Range:

$38.79 - $41.06 per hour, based on longevity at Frontier Behavioral Health.
