Job Summary

Frontier Behavioral Health is seeking a skilled Clinical Care Coordinator to join our team. This role will provide clinical services to patients admitted to our Evaluation and Treatment (E&T) inpatient units, coordinating and planning patient discharge, and connecting clients to various resources.

Key Responsibilities
  • Provide case management services, including coordination of housing, Medicaid enrollment, and insurance enrollment.
  • Facilitate psychoeducational groups as part of the therapeutic treatment plan.
  • Provide outreach and follow-up to individuals discharged from the Evaluation and Treatment Facilities.
  • Coordinate with various mental health and chemical dependency treatment agencies.
  • Complete a psycho-social intake with each client.
Requirements
  • Master's degree in the behavioral sciences.
  • Active BLS within 90 days of hire and must maintain active BLS.
  • Reliable transportation, insurance, and an active driver's license.
